Introduction to Aircraft Accident and Incident Investigations, Enrichment Syllabus (version 13 Feb 2020)

LEARING OBJECTIVE
The introduction course is intended to provide participants with the basics to perform an investigation on safety related issues within their
organisation.
TARGET GROUP
The target audience for this course are the persons in organisations
who have to perform the investigations of the incidents in their organisation, like:
  • Safety officers, Flight Data Analyst
  • Nominated Persons as per EU- reg. 965/2012
  • Investigators for AOC, NCC, NCO holders, business- planes /- jets and airlines
  • Investigators for ATO, Approved Training Organisations
  • Investigators for HEMS operators, Helicopter Emergency Medical Services
CONTENT
The following subjects will be introduced during the course:
  • ICAO Annex 13, applicable EU Regulations, the legal requirements for incident investigations
  • Stakeholders
  • The difference views on investigations (ideas from history starting with Heinrich’s iceberg via the Swiss Cheese model of prof.James Reason, to the ideas of prof.Erik Hollnagel, FRAM and prof.Sidney Dekker as the latest views on safety investigations)
  • Sources to be used during investigation
  • Data collection, root cause analysis
  • Human factors (decision making, environmental, stress, situation awareness, fatigue, communication)
  • Interviews techniques
  • Analysis of the collected data
  • Conclusions, including information on causes, contributory factors, risk and severity ➢ Databases of incidents and recommendations and the use of databases
  • Report writing
  • Files
EXAM

Pass mark: 75% or higher, course completion certificate will be issued.
